This promotional grocery rebate certificate allows the recipient to receive up to the value of the certificate in food items or merchandise at the grocery brand of their choice, in increments of $20 rebated in gift card form for every $100 they spend monthly.

Terms and Conditions

1. When you receive your certificate go on line to www.monthlygiftcards.com and enter the serial number on the
certificate so that you may register. You must register your certificate prior to the registration deadline printed on the
certificate.
2. During the registration process, you must answer each question including the name of your selected grocer/grocery
store chain from which you will collect receipts during the redemption period. Only grocery receipts from your selected
grocery store qualify. Grocery receipts from other grocers do not qualify.
3. Once you have completed the registration process, you are officially registered into the Monthly Gift Card Program.
To activate your registration, you will be asked to print and complete the Registration Validation Form. Mail and post
mark it along with your refundable registration fee of $12.95 in the form of a money order, or cashier’s check (no US
Postal Money Orders please) made payable to Star Rebates Inc. within ten (10) calendar days from the date you printed
out your online Registration Validation Form.
4. If you fail to enclose proper payment and postmark your envelope within ten calendar days from the date printed
on the Registration Validation Form you printed out, then your activation will be denied. Upon denial, the refundable
registration fee will be returned to you.
5. Only one certificate per type (food, fuel, retail, etc.) per household per year is permitted. If more then one voucher
type is submitted, only the first voucher is accepted.
6. If you are activating multiple certificate types (food, fuel, retail, etc.) at the same time each certificate must be treated
separately and you cannot combine Registration Validation Forms or registration fees into one envelope. Mailing multiple
registrations in one envelope or combining registration fees for two or more certificates onto one cashiers check or
money order, will result in the disqualification of each certificate being submitted.
7. After you mail your Registration Validation Form, be sure to check the Member Section at www.monthlygiftcards.com
to confirm your certificate has been activated. Once on the site, click on “Activation Status” to receive confirmation as
to whether or not your certificate has been activated.
8. When checking Activation Status, it is important to check the status before the first day of the month as the first day of
the month marks the first date in which you can begin collecting receipts from your selected grocer if you are approved.
When you check your status, if you are “Declined”, there will be an explanation and the refundable registration fee shall
be refunded. If you receive a “Pending” notice, that simply means that your account is not activated yet and you will
need to check before the first day of the next month. If you receive an “Approved” status, you may begin collecting receipts
starting in the month in which you have been approved. Please note that it may take up to 3 – 5 weeks to update
your status so if your status is pending, please be certain to check back before the first day of the next month so that
you have the optimal opportunity to collect receipts and reach the minimum receipt total required.
9. Once your activation is approved, you can begin collecting receipts for the month in which you are approved. When
collecting receipts for your selected grocer/grocery store chain, your receipts for that month must total a minimum of
$100 and the receipts must be dated within the same month in which you are eligible to obtain your rebate. Each receipt
must be original and legible and must include the grocery store name and date.
10. Each receipt must be comprised of a minimum of 75% food items. Food items include human food, pet food and
non-alcoholic beverages. Any receipt that contains less then 75% coming from the above mentioned items will not be
accepted. You can have multiple receipts from each month that total your $100 minimum excluding taxes.
11. Prior to but no later than the last day of the month, you must return to www.monthlygiftcards.com and print out a
Monthly Voucher Form for that month. Unfortunately, you will be disqualified for that month only, but can participate in
the program the next month, if you forget to print out your Monthly Voucher Form by the last day of the month. To print
out the voucher, you simply enter the Members Section of the website and click on Monthly Voucher Calculation. A
$100 target of qualified purchases must be achieved in order to print your monthly rebate voucher for the month. Only
print out the monthly voucher for the current month if you receive a “preliminary approval.”
12. While in the Monthly Voucher Calculation section you will be asked to complete the current Monthly Voucher Form
that will be submitted with your receipts. The form must be completed online before you print it out and submit it with
your receipts. When completing the Monthly Voucher Form, you will be asked to include your basic personal data and
to then itemize each of your receipts being certain to list the date of each receipt, the name of the grocer, the total of
each receipt, the sub total of each receipt that was spent on groceries and the sub total of each receipt that was spent
on non-qualifying items. Only receipts matching the same month as the monthly vouchers shall be accepted.
13. After you have completed the online Monthly Voucher, press submit to get a preliminary status of your monthly
rebate. If you have made an error, you will be able to recalculate that month’s submission and resubmit it. After you hit
submit, there will either be “preliminarily approved” or “declined.” If you are declined, you will receive an explanation
and you will then have the opportunity to submit again the next month. To close out the Monthly Voucher Calculation
Section, you will need to click “Accept” or “Accept/Print.”
14. If you are “preliminarily approved” you will be asked to print out the Monthly Voucher Form and submit it with your
receipts by the 15th day of the next month. Failure to postmark the envelope on or before the 15th day of the following
month shall result in a reversal of your “preliminary approval” and this will be reported to you in the Members Section of
www.monthlygiftcards.com under the heading Monthly Gift Card Status.
15. Upon receipt of your Monthly Voucher Form and receipts, your submission will be double-checked and the status
reported in the Members Section under Monthly Gift Card Status. If you intentionally deceive us by improperly completing
your Monthly Voucher Form, you shall be disqualified from the program and there will be no opportunity to resubmit
the remainder of your certificate vouchers.
16. Within 30 days of receipt of your Monthly Voucher Form and receipts, the final disposition of that months voucher
will be reported on www.monthlygiftcards.com in the Monthly Gift Card Status section within the Members Section. In
checking the status, you will be either approved, declined, or disqualified with an explanation.
17. Monthly Voucher Forms and monthly receipts must be submitted in their original form and must be for the same
matching month. Vouchers or receipts that are not originals or are from the wrong months will not be accepted.
18. This certificate is valid for the number of months specified by the value of the certificate divided by $20. These
months run consecutively and without interruption.
19. If your Monthly Voucher Form is accepted for any given month, you will receive your rebate for that month. The
rebate will be processed in one of two ways: a $20 gift card will be sent within 30 days from the date your Monthly
Voucher Form was approved as listed in the Monthly Gift Card Status Section; or we will provide you with a credit
to your visa account within 1 business day from the date your Monthly Voucher Form was approved as listed in the
Monthly Gift Card Status Section. After you submit your final Monthly Voucher Form, and redeem each of the monthly
vouchers, your final monthly rebate will contain the $20 for that month plus an additional $12.95 equal to the original
$12.95 refundable registration fee.
20. When you receive your monthly gift card, you will have 90 days from the date you receive it, to use it. After 90
days, a $5 monthly maintenance fee is deducted from the monthly gift card until the full value of the monthly gift card
is depleted.
21. Monthly Voucher Forms have no cash value and will not be accepted by the grocery store chosen by you during
the registration process.
22. Monthly Voucher Forms and receipts may only be redeemed by postmarking them in a timely fashion as detailed
above in these Terms & Conditions.
23. The refundable registration fee becomes non-refundable if these Terms & Conditions are not followed.
