Terms and Conditions
Congratulations, your sponsor has awarded you with this cruise redemption certificate redeemable for a cruise voucher entitling you and a companion your choice of a three, four, five, or six day standard cabin accommodations aboard a choice of luxury cruise ships.
To redeem this offer you must activate it within 30 days of receiving by completing and submitting this form either by mail; Premium Fulfillment c/o Cruise Redemption PO Box 10627 Daytona Brach, FL 32120-0627, Fax (386) 255-0900, or online at www.cruiseredemption.com along with $14.50 per person ($29.00) to cover the cost of shipping, handling and processing. Once this cruise redemption certificate is activated you will receive via USPS a cruise selection and reservation request package along with complete details on completing your booking. Your cruise selection and reservation request package is valid for one year from the time of activation and all cruise bookings must be finalized a minimum of 90 days prior to departure.
Cruise voucher is for interior stateroom accommodations on the entry level deck for two guests. Changes in accommodation type may be available upon request at an additional cost. Additional guests must be purchased from Executive Tour & Travel Services, at available Cruise Line rates. Guests under the age of 21 years must be accompanied by a parent, grandparent or a guardian 25 years or older in the same stateroom. This certificate covers cruise cabin fare only. You will be responsible for all other expenses, including without limitation: airline tickets, airport transfers, ground transport, taxes, government imposed security fees, miscellaneous personal and incidental expenses, alcoholic beverages, spa services and gratuities, shore excursions, administrative costs and agency fees. Government taxes and fees are additional for all guests and are payable at the time of booking confirmation. Fuel supplement charges may be required on some bookings.
